CCO 101: Understanding Coordinated Care in Oregon

Whether you're a case manager, community health worker, social service provider, or anyone who supports Oregon Health Plan members, this session is for you.

The Community Services Network (CSN) is bringing together Portland's two Coordinated Care Organizations, Trillium Community Health Plan and Health Share of Oregon, for an interactive CCO 101 learning session. If you've ever found yourself wondering how CCOs actually work, or wanted to better connect the people you serve with the right care and resources, this is the session you don't want to miss.

Representatives from both health plans will walk you through how their organizations operate, what benefits and services they offer, and how members access care and support, straight from the source.

What you'll walk away knowing:

  • What Coordinated Care Organizations (CCOs) are and how they fit into Oregon's healthcare system

  • How CCOs coordinate physical, behavioral, and dental health services under one umbrella

  • How Trillium and Health Share each structure their benefits, programs, and services

  • How members navigate and access care through their CCO

  • Where to turn for resources, referrals, and ongoing support

You'll also have dedicated time to ask questions and get real answers from the people who know these systems best.
